Faculty Hiring Policy & Forms

For faculty members to be hired, they must adhere to the policies in place, fill out the appropriate forms and provide an official transcript: 

  • FACULTY TEACHING QUALIFICATIONS POLICYFaculty Teaching Qualifications Policy
  • PERSONNEL ACTION FORM:  Select the Personnel Action Form on the Human Resources forms web page.
  • OFFICIAL TRANSCRIPTS:  The University requires an official transcript of the faculty member's terminal degree as it relates to the teaching area.  Transcripts should be sent by EScript to faculty.transcripts@louisiana.edu -OR- Mail to: Dr. Dianne F. Olivier, Associate Vice President for Faculty Affairs; Academic Affairs; Univ. of Louisiana at Lafayette; P.O. Box 43589; Lafayette, LA 70504-3589
  • FACULTY CREDENTIALS CERTIFICATION FORM:  Additional professional experience may be considered, in addition to awarded degrees, when reviewing credentials for potential faculty.  The Faculty Credentials Certification Form must be completed by the Department Head and submitted to the College Dean for approval.  Upon approval from the Dean, the form is then routed to Dr. Dianne F. Olivier, Associate Vice President, Academic Affairs, Office of Student and Faculty Excellence for final review and approval.
